上海2026年10月23日 /美通社/ -- 禮邦醫藥（江蘇）股份有限公司（以下簡稱「禮邦醫藥」或「公司」，股票代碼：09637.HK），一家聚焦腎臟疾病領域的生物醫藥企業，今日宣布其合作夥伴 R1 Therapeutics, Inc.（以下簡稱「R1」）已於當地時間 2026 年 10 月 22 日在美國科羅拉多州丹佛舉行的 2026 年美國腎臟病學會（ASN）腎臟周（ASN Kidney Week 2026）上，以進行中臨床試驗（trial-in-progress）壁報形式展示了同類首創口服泛磷轉運蛋白抑制劑 AP306 用於接受維持性血液透析的高磷血症患者的全球 IIb 期多區域臨床試驗（NCT06712654）的研究設計。該項全球 IIb 期多區域臨床試驗由禮邦醫藥與 R1 共同申辦，目前正在美國及中國的多個臨床中心開展。 該項全球 IIb 期多區域臨床試驗（NCT06712654）是一項多中心、隨機、雙盲、安慰劑對照固定劑量研究，旨在評估 AP306 的安全性、耐受性及降低血清磷的效果。研究計劃入組約 168 名接受維持性血液透析的高磷血症受試者，隨機分配至 6 個 AP306 固定劑量方案組及 1 個安慰劑對照組，治療期為 8 周；主要終點為血清磷水平自基線至治療結束時的變化，次要終點包括達到目標血磷範圍的受試者比例及首次達到血磷控制的時間。本研究採用多劑量平行組設計，旨在系統闡明 AP306 的劑量-效應關係，並同步評估每日兩次（BID）與每日三次（TID）不同給藥頻次下的療效、安全性及耐受性特徵，從而進一步優化給藥方案、減輕透析患者的日常服藥負擔並提升長期用藥依從性，為後續臨床開發的劑量與給藥方案選擇提供關鍵依據。該試驗中國區牽頭研究中心為北京大學人民醫院，共包括 7 家臨床中心，已獲中國人類遺傳資源管理辦公室批准；公司已於 2026 年 7 月 21 日就該試驗首例受試者完成隨機化並給藥發布公告，目前美國及中國臨床中心的受試者招募正在進行中，預計試驗將於 2027 年第二季度完成，頂線數據將在適當時候公佈。 AP306（前稱 EOS789）是一款同類首創的口服泛磷轉運蛋白抑制劑，最初由日本中外製藥股份有限公司（以下簡稱「中外製藥」）發現，禮邦醫藥獲授權引進並取得其全球獨家開發、生產及商業化權利。與在腸道腔內被動結合游離磷的傳統磷結合劑不同，AP306 通過同時抑制三個關鍵的鈉依賴性腸道磷轉運蛋白——鈉依賴性磷酸鹽轉運蛋白 IIb（NaPi-IIb）、磷酸鹽轉運蛋白 1（PiT-1）及磷酸鹽轉運蛋白 2（PiT-2），主動阻斷腸道對磷的主動轉運吸收。作為全球首個且唯一進入臨床開發階段的泛磷轉運蛋白抑制劑，亦是唯一同時靶向上述三個關鍵腸道磷轉運蛋白的口服候選藥物，AP306 有望在提供顯著降磷療效的同時大幅減輕患者的日常服藥負擔，為高磷血症管理提供差異化的治療路徑。 未被滿足的臨床需求 高磷血症是接受透析的慢性腎臟病（CKD）患者常見且重要的併發症，約 95% 的透析依賴型慢性腎臟病患者受累。[1-3] 血清磷水平升高與血管鈣化、繼發性甲狀旁腺功能亢進、腎性骨病以及心血管事件和全因死亡風險增加相關。[1-3] 規律透析及飲食限磷通常不足以有效控制磷負荷，患者往往需要長期藥物治療；即便廣泛使用磷結合劑，中國仍約有 76% 的透析患者血清磷水平未達到 1.13～1.78 mmol/L（3.5～5.5 mg/dL）的目標範圍。[4] 另據透析預後與實踐模式研究（DOPPS）報告，2021 年美國和日本中心血液透析患者中血清磷高於 1.78 mmol/L（5.5 mg/dL）的比例分別約為 52% 和 39%（不同研究的人群、年份及指標定義不同）。[5] 中國透析患者規模在 2025 年約為 130 萬人，預計 2035 年增至約 340 萬人（複合年增長率約 10.1%），未被滿足的臨床需求持續擴大。[6] 現有磷結合劑療法仍普遍面臨單片劑量大、每日服藥片數多、胃腸道耐受性受限及長期依從性不足等挑戰，兼顧降磷療效、耐受性與用藥便利性的創新治療選擇仍有待進一步開發。[1-3, 7] 參考文獻 [1] Kidney Disease: Improving Global Outcomes (KDIGO) CKD-MBD Update Work Group.
